Terms and conditions for Maternity Leave Membership

This contract is made between the client and FIT4MOM South Bay and is valid for Maternity Leave Membership at FIT4MOM South Bay classes. Membership is for 6 weeks of unlimited FIT4MOM classes. Membership is non-refundable and non-transferrable.

I authorize FIT4MOM South Bay (ASC Fitness & Nutrition) to initiate a charge to the account entered into Pike 13 for my classes.

No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the South Bay franchise.
