Terms and conditions for Monthly Unlimited Membership

This contract is made between the client and FIT4MOM South Bay and is a month to month agreement (after the initial 3 month commitment) and except as provided in the initial 10 day cancellation policy below, may be terminated by either party with the proper advance notice. There will be an early cancel fee of $99 processed if cancelled before the 3 month commitment. This membership allows you to attend unlimited Fit4Mom South Bay classes.

AUTHORIZATION FOR PAYMENT:

I authorize FIT4MOM South Bay (ASC Fitness & Nutrition) to initiate a charge to the account entered into Pike 13 for my dues. This authorization is to remain effective until I have cancelled my membership in writing through the FIT4MOM South Bay administrator. I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM South Bay to fulfil my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

CANCELLATION POLICY

To cancel this membership, the member must contact the FIT4MOM South Bay administrator at southbayla@fit4mom.com at least 30 days prior to their next payment due date to request a cancellation. Cancellations will be confirmed in writing to the member once they have been processed. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the South Bay franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee. Instructors are not authorized to cancel memberships.

HOLD POLICY

Memberships may be held a maximum of 4 months per hold period. To place a hold on this membership, the member must contact the FIT4MOM South Bay administrator at southbayla@fit4mom.com at least 10 business days prior to their next payment due date to request a hold be placed on their membership. Holds will be confirmed in writing to the member once they have been processed. Hold starts will coincide with the next date dues is scheduled to be drawn out on and can end on any day of the month. When a hold period is ended, the monthly auto payments will automatically resume on the return date. It is up to the client to keep track of their return date. There will be a $20 processing fee charged to your credit card on file for the hold period. Instructors are not authorized to place a hold on memberships.

INITIAL 10-DAY CANCELLATION POLICY

You, the buyer, may cancel this agreement at any time prior to midnight of the tenth (10th) calendar day after you have signed this agreement. To cancel this agreement written notice must be received by FIT4MOM South Bay within the initial ten days. You are responsible for paying for the classes already attended.
